Why Your Hip Pain Might Not Be a Hip Problem
The most consistent finding we see with hip-region pain is a restricted sacroiliac (SI) joint. When the SI joint loses normal mobility, the hip flexors, TFL, and gluteal muscles tighten to compensate, and over time that tension becomes pain you feel directly in the hip. The problem is one level up, and it has never been examined.
Every hip pain assessment at Full Swing includes the SI joint, lumbar spine, and hip joint mechanics, not just the area that hurts. If you have been diligently stretching your hip flexors for months without improvement, there is almost certainly a mechanical driver that stretching alone cannot touch.
Deep Structures That Need More Than Manual Pressure
Two muscles cause a lot of hip pain that never resolves with conventional care:
- Iliopsoas: the deepest hip flexor, sitting just in front of the lumbar spine. When it develops active trigger points it refers pain into the groin, anterior thigh, and low back. External massage cannot reliably reach it. Dry needling can.
- Piriformis: tucked deep in the posterior hip. When it shortens from trigger point activity it can compress the sciatic nerve, producing pain that travels down the leg. Again, dry needling is the most direct clinical access available short of a cortisone injection.

- Orthopedic and neurological testing to rule out referred pain from the low back
- Muscle-length and trigger point assessment of the iliopsoas, piriformis, TFL, and glutes
- Same-day treatment when appropriate, most patients leave the first visit feeling a difference
