The SI Joint and the Hip

The most consistent finding with hip-region pain is that the sacroiliac joint is involved. A restricted SI joint creates compensatory tension in the hip flexors, TFL, and gluteal musculature that eventually becomes painful. The pain is felt in the hip area. The source is a joint one level up that has never been examined.

We assess the SI joint, lumbar spine, and hip joint mechanics in every hip pain presentation. Not just the area that hurts. The patient who has been stretching their hip flexors daily for six months without improvement almost always has a mechanical driver that stretching cannot address.

Deep Structures That Need More Than Manual Pressure

The iliopsoas is the deepest hip flexor and it refers pain into the groin, anterior thigh, and low back when it harbors active trigger points. External manual therapy cannot reach it effectively. Dry needling can. The piriformis in the posterior hip compresses the sciatic nerve when it shortens from trigger point activity. Same situation. These are structures where the needle is the most direct clinical access available short of injection.

A Specific Patient

A cyclist from Riverside had right hip pain that had been building over a training block. Daily foam rolling, daily stretching, no meaningful improvement. When Dr. Muren assessed him, the right TFL, iliopsoas, and piriformis all had active trigger points that had developed from sustained hip flexion in the cycling position. The rolling had been compressing those trigger points temporarily without releasing them. We needled all three in one session. The lateral hip pain quieted within 24 hours. Two more sessions cleared the remaining pattern.
