The Infraspinatus and Teres Minor

The infraspinatus and teres minor sit on the posterior shoulder blade and refer pain into the anterior shoulder and lateral arm. Active trigger points in these muscles create a pain pattern that feels like it is coming from the front of the joint when the source is in the back. Patients have typically had anterior-focused treatment for months with incomplete results.

Acupuncture accesses the posterior rotator cuff at the depth of the tissue. The local response and subsequent release change the referral pattern significantly. Patients often feel the anterior shoulder pain diminish during the session itself as the posterior source releases.

The Cervical Contribution

We assess the cervical spine in every shoulder pain presentation. Restrictions at C5-C6 directly refer into the shoulder and arm. Scapular mechanics are altered by thoracic restrictions. Treating the shoulder without addressing these contributors produces shorter-lasting results because the shoulder girdle continues to load abnormally between sessions.

Frozen Shoulder

Adhesive capsulitis responds to acupuncture in a specific way. The autonomic pain sensitization that makes the capsular restriction so painful can be modulated through acupuncture, which makes the stretching and mobilization work more tolerable and therefore more aggressive. Patients who find the mobilization work too painful to progress often tolerate it significantly better when acupuncture is part of the treatment plan.
