Where it hurts tells us a lot

Point to the spot and describe when it hurts, and you have usually narrowed it down to one of a few common problems:

  • Below the kneecap: patellar tendinopathy, or jumper’s knee. Pain right at the bottom of the kneecap that flares on stairs, after a long run, or when you stand up from a chair.
  • Around or behind the kneecap: runner’s knee. A dull ache that builds with squatting, stairs, and sitting for a long time with the knee bent.
  • Outside of the knee: IT band syndrome. A sharp or burning spot on the outer knee that comes on at a predictable point in a run or a ride.
  • Inside of the knee: often a strained ligament, an irritated tendon where it attaches, or a meniscus that has been overloaded.
  • Deep and stiff: wear in the joint itself, usually worse first thing in the morning and after sitting, easing once you get moving.
  • Catching or locking: a knee that clicks and catches, or suddenly will not straighten, can point to a meniscus problem.

Why the knee is usually the victim

The knee is a hinge caught between two joints that move in every direction. When the hip is weak or stiff, the thigh drops inward with every step and drags the kneecap off its track. When the ankle is stiff or the foot flattens, the shin rotates and twists the knee from below. The knee takes the punishment either way, which is why it is so often the joint that hurts and so rarely the joint that started it.

So every knee exam here includes the hip, the ankle and how you move: a single leg squat, a step down, and for runners, a look at stride. Treating the IT band without looking at the hip is treating the symptom, not the problem. Fix the movement, then clear the tissue, and the result lasts.

Tendon pain that stopped getting better

Tendon problems at the knee have a clock. In the first weeks, a structured loading program usually does the trick. After three or four months without real progress, the tendon shifts from an irritated state to a degenerative one, and exercises alone rarely finish the job.

Adjustments above and below the knee

Dry needling

When a knee needs imaging first

Some knees need a look inside before anyone treats them. A knee that swelled up fast after a twist, one that gives way under you, one that locks and will not straighten, or pain after a real fall or collision can mean a torn ligament or meniscus. If your exam points that way we will refer you for imaging or an orthopedic opinion first. Wear in the joint is a different conversation again: some arthritic knees respond well to conservative care and some are not a good fit, and we will tell you which yours looks like before you commit to anything.

Keep moving within the range that does not hurt, because complete rest weakens the muscles the knee depends on. Cut back your mileage or your jumping instead of stopping everything, and avoid deep squats and long downhill runs until we have looked at you. Ice helps a hot, swollen knee after activity. If the knee is locking, giving way or swelling fast, do not wait for your appointment; get it checked the same day.

How do I know if it’s runner’s knee or jumper’s knee?

Jumper’s knee hurts right at the bottom of the kneecap, on the tendon. Runner’s knee is a duller ache around or behind the kneecap that builds with stairs, squats and sitting. The exam confirms which one you have, and the treatment is different for each.

Can shockwave therapy help knee pain?

Do I need an MRI first?
