Here is what the machines do, why we went a different direction for the same problems, and what we use instead.

What cold laser therapy actually is

Cold laser goes by several names: low level laser therapy, LLLT, photobiomodulation, and in its higher powered form, Class IV laser. A handpiece delivers light at particular wavelengths into the tissue. It does not cut and it does not heat the tissue meaningfully, which is where the word cold comes from. Most people feel nothing at all during a session, or mild warmth.

The proposed mechanism is real biology rather than marketing. Light at certain wavelengths is absorbed by structures inside the cell, which is thought to nudge cellular energy production and shift the local inflammatory chemistry. The argument for it is that this speeds up what the tissue was already trying to do.

Why we do not offer cold laser at our Jacksonville clinic

We are not claiming it does nothing. Photobiomodulation has genuine research behind it and some of that research is positive. Our objection is about how reliably you can get the effect, and about what else that same appointment slot could be doing for you.

  • The dosing is where it falls apart. Outcome depends on wavelength, power, energy delivered, time on the tissue and depth of the target, and studies use wildly different combinations. That is why the reviews come back inconsistent: not because light does nothing, but because “laser therapy” describes dozens of different treatments wearing one name. When a treatment’s results depend that heavily on settings nobody standardises, you cannot promise a patient much.
  • Depth is a real constraint. Light scatters and is absorbed as it travels, so the deeper the target the less arrives. Superficial structures get a reasonable dose. A hip, a deep glute insertion or a lumbar disc gets progressively less, which is often exactly what the patient is being treated for.
  • It is passive, and it is sold in packages. The machine is expensive, so it tends to be recovered through ten or twenty session packages bought up front, usually cash because insurance rarely covers it. Meanwhile nothing about lying under a light changes how you load the tissue, which is what decides whether the problem returns.
  • We already chose a deep tissue tool, and we chose a stronger one. For the exact problems laser is aimed at, stubborn tendon and fascia cases, we run focused shockwave instead. It is a mechanical input rather than a light one, the dose is countable in pulses, and it reaches depth that light cannot.

What we use instead, and why it goes deeper

The machine we did buy is a StemWave, and it is focused electrohydraulic, which is the most specific and deepest penetrating type of shockwave available. That matters more than it sounds. Most clinics advertising shockwave run radial pressure wave devices, which spread their energy out from the applicator so the effect is strongest at the skin and fades quickly with depth. Focused energy converges at a set depth instead, so what you are treating actually receives the treatment.

The other difference is what it does. Shockwave is a mechanical stimulus, and tissue reads mechanical force and responds to it, a process called mechanotransduction. That is the same reason loaded bone gets denser and a tendon rebuilds around demand.
