Paperwork & check-in
You’ll fill out a short health history covering your symptoms, past injuries, and goals. Want to save time? Complete it online through our ChiroTouch patient portal before you arrive. If you’re using insurance, bring your card and a photo ID so we can verify your benefits.

Sit-down consultation
One of our doctors talks through what’s bothering you, when it started, and what makes it better or worse. This is your time to ask questions. Nothing happens to your body until you understand the plan and you’re comfortable with it.

Full assessment & exam
We test range of motion, posture, strength, reflexes, and the specific joints and muscles involved. This hands-on evaluation is how we tell, say, a pinched nerve from a tight muscle, so we treat the cause instead of chasing the symptom.

Diagnosis & custom plan
We explain in plain English what we found and how long we expect recovery to take. Your plan may combine chiropractic adjustment with acupuncture, shockwave, dry needling, cupping, or massage, drawing on the whole toolbox under one roof.

- Wear comfortable, loose-fitting clothes you can move in.
- Bring your insurance card and a photo ID if you’re using benefits.
- Bring any recent imaging (X-rays or MRI) or referral notes if you have them.

What it costs
We keep pricing transparent. Self-pay is $115 for the first visit (the full exam is included) and $50 for follow-up adjustments. We accept all major credit and debit cards and cash. We also accept and bill Florida Blue, United Healthcare, Humana, Cigna, Florida Medicaid, and Veterans Affairs (VA), so for many patients out-of-pocket cost is even lower. How long will my first visit take?
Plan for about 45 to 60 minutes. That covers your consultation, a full exam, your diagnosis and plan, and usually your first treatment the same day.
Do I need a referral to become a patient?
No referral is needed to book with us directly. If you’re using certain insurance plans, check whether your plan requires one; we’re happy to help you verify your benefits over the phone at (904) 539-3352.
Will I get adjusted on the first visit?
In most cases, yes. Once we’ve completed the exam and you’re comfortable with the plan, we typically begin treatment the same day so you start feeling better right away. If your situation calls for a more cautious approach, we’ll tell you.
What should I wear and bring?
Wear comfortable, loose clothing you can move in. Bring a photo ID, your insurance card if you’re using benefits, and any recent X-rays, MRI results, or referral notes you may have.
