You need a DOT physical if you operate any of the following in interstate commerce:
Vehicles with a gross vehicle weight rating of 10,001 pounds or more
Vehicles designed to transport 9 or more passengers including the driver for compensation
Vehicles designed to transport 16 or more passengers including the driver not for compensation
Vehicles of any size transporting hazardous materials requiring placards

A DOT physical is a comprehensive evaluation of your fitness to safely operate a commercial vehicle:
Vision test: must meet minimum acuity standards with or without corrective lenses
Hearing test: must perceive a forced whispered voice in the better ear
Blood pressure and pulse: evaluated according to FMCSA guidelines
Urinalysis: screens for diabetes and kidney function, not a drug screen
Physical examination: cardiovascular, respiratory, musculoskeletal, and neurological systems
Health history review: any conditions that could affect driving safety

A DOT medical certificate is valid for up to 24 months for drivers who meet all health standards without conditions. Drivers with certain conditions like controlled hypertension or diabetes may receive shorter certifications of 6 or 12 months and need more frequent exams to stay compliant.
Having a medical condition doesn't automatically disqualify you from commercial driving. Controlled hypertension, well-managed diabetes, certain vision or hearing limitations.
